R-Kids Enrollment

Welcome to Restoration Kids!
We’re so excited to partner with you in helping your child grow in faith, friendships, and fun! This form gives us the information we need to create your family’s profile in our check-in system and to get your child set up for R-Kids ministry. Your details help us keep your child safe, cared for, and connected during services and events.

Household Adult Information

Trusted Adults

A trusted adult” is anyone you authorize to check in or pick up your child from Children’s Ministry. This could be a grandparent, close family member, or a family friend you trust.


Non-Authorized Persons

For your child’s safety, you may list any individuals who are not permitted to check in, pick up, or have access to your child while in Children’s Ministry. This ensures our team knows who should not be in contact with your child.

Policies Acknowledgment

Sick Child Policy
 
For the health of all children, please do not bring your child if they have a fever over 100 , vomiting, diarrhea, rash other than from teething, or other contagious illness. If your child becomes sick while in our care, we will contact you immediately to pick them up.
 
Behavior Policy
 
Children are expected to be kind and respectful to volunteers, staff, helpers, and other children. If behavior becomes unsafe or disruptive, parents will be contacted to come and assist or pick up their child.
 
Late pickup Policy
 
Parents must pick up their child promptly when the church held event ends.  If a parent or guardian has not arrived within 20 minutes after the church even ends, the listed emergency contact will be called.
 
Supplies Policy
 
Please bring any items your child may need (bottles, diapers, wipes, change of clothes, etc.). The church does not provide these items. For the safety of all children, any snacks brought from home must be peanut-free.

Parent/Guardian On-Site Policy

For the safety and well-being of all children in our care, Restoration Church requires that a parent or guardian remain on church premises at all times while their child is participating in Children’s Ministry. Parents/guardians also agree and understand that they may be contacted via text message or phone call if they are needed for any reason regarding their child during service.

Snack and Beverage Policy

Restoration Church provides a structured snack time during Children’s Ministry services. By enrolling my child, I consent to Restoration Church providing a snack and beverage for my child unless I choose to provide one myself.

Restoration Church Children's Ministry Liability Release

As the parent/guardian of the child(ren) listed on this form, I give permission for my child(ren) to participate in the Children’s Ministry programs and activities of Restoration Church during Sunday services.
I hereby release, discharge, and hold harmless Restoration Church, its pastors, directors, volunteers, and representatives from any and all liability, claims, or demands for personal injury, sickness, or property damage that may occur while my child(ren) is/are under their care.
I also give consent for emergency medical or dental treatment in the event of an accident or illness while my child(ren) is/are participating in Children’s Ministry, including examination, diagnosis, treatment, anesthetic, or surgical care, and I agree to be responsible for any expenses incurred.

By checking this box, I give Restoration Church permission to use photos or videos of my child(ren), taken during Children’s Ministry activities, for church purposes (such as publications, social media, and promotional materials).

By not checking this box, I withhold permission for my child(ren)’s image to be used.
